Risk Bow-Tie analysis
The Bow-Tie diagram is a structured risk visualization technique that maps, in a single view, everything that can go wrong (causes), the risk event itself, and everything that follows from it (consequences) - together with all the controls and measures in place on both sides. In Omega 365, the Bow-Tie is built entirely around Scope Items. Each cause, consequence, preventative measure, and mitigating measure in the diagram is a fully functional Scope Item.

For risk managers, this brings several key benefits:
Clarity — Instead of reading through separate lists of workflows, assessments, and consequence areas, the Bow-Tie presents the full risk picture at a glance, making it easier to communicate risk to stakeholders and decision-makers.
Control visibility — Preventative and mitigating measures are shown in their correct position relative to the risk event, making gaps in coverage immediately apparent.
Auditability — Because every element of the diagram is a live, linked record, the diagram always reflects the current state of the risk without any manual maintenance.
Accountability — Each element is owned, tracked, and maintained through its own process, ensuring responsibility is clear across the Bow-Tie.
In Omega 365, the Bow-Tie is built entirely around Scope Items. Each cause, consequence, preventative measure, and mitigating measure in the diagram is a fully functional Scope Item - a workflow, activity, meeting or follow-up process with checklists, verifications, actions - configured and maintained according to the process it belongs to. This means every element carries its full content: attachments, automations, status tracking, responsible persons, checklists, and any other data defined by that process type. The Bow-Tie diagram simply surfaces these existing records in a visual structure, without creating any parallel or duplicate data.
Opening the Bow-Tie View

Navigate to a risk record and open the Activities & Workflows section.
At the top of the section, you will see two toggle buttons: List and Bow-Tie.
Click Bow-Tie to switch to the Bow-Tie view. The tab panel below will automatically switch to the Bow-Tie Preview tab.
Click List to return to the standard grid view.
When in Bow-Tie view, you can also click on each Scope Item and view it's content in Details tab below.
Setting Up the Bow-Tie
Before the diagram can be populated, workflows and activities must be assigned to a Bow-Tie area using the Bow-Tie grid (visible when the Bow-Tie toggle is active).
Each linked scope item can be assigned to one of four areas:

Preventative | Measures that reduce the likelihood of the risk occurring |
Mitigating | Measures that reduce the impact if the risk occurs |
Consequences | Outcomes or impacts resulting from the risk event |
Preventative and mitigating measures can also be linked to a specific Cause or Consequence respectively, which will draw a connecting line between them in the diagram.
Default Processes per Area
Default processes for each Bow-Tie area can be pre-configured by an administrator in Omega 365 Setup → Bow-Tie Setup. When configured, users creating new scope items from within the Bow-Tie will be offered the relevant promoted processes for that area, reducing setup effort and ensuring consistency across risks.
User can also selected any other Scope Item already related to the risk to be included in the Bow-Tie diagram. System also provides an indication (warning) if specific Scope Item is not of the same process type as defined per Bow-Tie Area setup.
Reading the Diagram
The central bubble represents the risk event itself, coloured according to the current assessment.
Left side — Causes flow into preventative measures, which connect into the central event.
Right side — The central event flows out to mitigating measures, which connect to consequences.
Each box is clickable and opens the linked scope item in a new tab.
Display Options
A small toolbar above the diagram provides three options:
Assessment Information
Toggles on/off (default: off) an information panel drawn directly on the diagram canvas below the Bow-Tie. When enabled, it shows:
Probability row — a coloured bar showing the current assessment label, date, and probability value.
Consequence lines — one plain text line per consequence area showing the area name and its assessed value.
Risk ID
Toggles on/off (default: off). When enabled, the central bubble displays the risk's unique ID instead of its title — useful when exporting or printing the diagram.
Full Screen
Click Full screen (top-right of the diagram) to expand the Bow-Tie to fill the entire screen. Click Exit full screen or press Esc to return.
Enabling Bow-Tie Functionality
The Bow-Tie feature is controlled by a site configuration setting. To enable it, a system administrator/tech. responsible must set the following site configuration key to true in Site Configuration:
Omega365:EnableRiskBowTie : true (enable HTML)
Once enabled, the Bow-Tie toggle will appear in the Activities & Workflows section of every risk record.
